Surface-mount prototyping adapters

A frequent topic of discussion on electronics mailing lists is how to go about prototyping with surface-mount parts. While there are a few maufacturers of adapters, they tend to be quite expensive -- far more than most other parts needed for prototyping -- and many people tend to roll their own adapters. This page is an attempt to collect together some ideas on how to do this, and links to surface-mount prototyping resources. If you have anything to add to this page -- links, designs, techniques, etc. -- please let me know.
